Use the form below to apply for a care package.
All applications are held for 30 days from when they are received.
One individual or family will be chosen every week from current eligible applicants.
Families may not receive more than one care package in any 30 day period.

Contents vary based on ages and interests, but common contents include reading books, small stuffed animals, small toys, coloring books, adult care items (lotion, candles, etc.), and a greeting card or other reminder that someone cares.

Care packages are delivered by hand.
Delivery addresses must be within 60 miles of Springfield, MO.
Selected families will not be notified prior to delivery of their package.
